That's how it works. But if it is common for people in your city to use an ISP with an ip address in another city, people in your city, if there are any here, would still show up as your neighbors.

That is defineatly  the case, but I doubt I can find anyone in my country even, with an account on this website, anyway a good initiative :)! I am trying to promote the site among my chess acquaintances, but it works better (for some reason) with my facebook/internetty friends rather than my "real" life friends :)!
